-This form is used to request the pickup and processing of IT equipment that is no longer in use and is ready to be removed from inventory. It ensures that devices are properly logged for surplus, sanitized if needed, and transferred in compliance with university policies. This form is typically completed by the IT Manager or their delegate and submitted to the Central Surplus team.
- The Intake Form is used to gather essential information about IT service needs, purchases, or issues. This helps the IT team accurately scope the request, prioritize tasks, and ensure alignment with budgeting and support policies. It is often the first step for faculty and staff when initiating IT-related projects or inquiries.
- This form is completed when a new system is being prepared for deployment. It gathers information such as the user's name, department, software requirements, preferred device configuration, and special access needs. It ensures that every system is properly configured before delivery and aligns with the College's standards and lifecycle policies.
