Move, Change, or Disconnect a Data or Voice Connection

This article describes how to make changes to existing data or voice connections, including relocating service, changing the service that the connection is configured for, or disconnecting an unused service.

Instructions

Please submit a Help Desk ticket or submit a “Move or Update an Existing Data or Voice Connection” form with the following information:

  1. Your department.

  2. Your budget code for billing the service.

  3. A brief description of the service you are requesting.

  4. If you are requesting a disconnect, please provide the building, room number, and jack number of the service to be disconnected.

  5. If you are requesting a change to an existing service, please provide the building, room number, and jack number of the service to be changed, and provide details of the change you want to make in the Description field of the form.

  6. If you are requesting a relocation of existing services, please indicate the building, room number, and jack number you are moving the service from, and the building, room number, and jack number you are moving the service to.

 

Changes, moves within buildings, and moves to different buildings are all charged to the budget code provided per the Departmental Fee Schedule. Please obtain approval from a budget authority before submitting these requests. Disconnects are provided at no charge.
